Transition to Stability is a coordinated re-entry support pilot serving adults returning to Southern Nevada from jail, prison, transitional custody, residential correctional programs, or other correctional settings. The program responds to the barriers that tend to arrive simultaneously at release: no safe address, no identification, limited income, no transportation, gaps in benefits and health coverage, employment barriers, behavioral-health needs, family strain, and limited connection to community services.

NDOC describes successful re-entry as including stable housing and employment, effective treatment, positive relationships and family support, and appropriate supervision. This pilot is designed to support those objectives through a voluntary, housing-centered community referral pathway.

Ideally 30–90 days before release; shorter timelines accepted when feasible

30 participants at one time; 6-10 referrals/enrollments per month

Pre-release planning plus up to six months of post-release case management, subject to participant choice and capacity

Safe release destination, housing stabilization, document recovery, benefits, transportation, workforce links, behavioral health/recovery referrals, basic needs, and peer support

Pre-release meetings only if NDOC approves; otherwise, referral, approved virtual/telephone contact, and post-release services
